The how2trak My Care app enables a patient receiving care for one or more wounds to perform and save a record of their own self-assessments, as well as to connect remotely in secure “virtual care” sessions with their clinical specialist.
The My Care mobile app interfaces with the how2trak web application to provide clinicians with a complete history of the patient’s wound progression, including photos, treatments, and plain-language assessments.
